Registration for the CDC program begins in January each year with the re-enrollment of current students and their siblings. Open registration dates will be publicized at that time, with priority being given to church members. A waiting list is maintained for those children that we are not immediately able to place.

Registration Requirements include:

• Completed registration form (click here to download printable forms)
• Non-refundable $50 registration fee
• Copy of current immunization records (New immunization records are required each year. State immunization waivers are accepted only for children two years of age and older.)
